footnotes.html contains a back-reference
<h2><a href="#fr2" name="f2">2</a></h2>
but it seems that #fr2 is in ch4.html, not footnotes.html. I wonder if
that link should be "ch4.html#fr2", similar to what the preceding "1"
back-reference has,
