Try to downgrade initramfs-tools to version 0.116 or below.
With version higher than 0.116 you might not be able decrypt a seperated /usr
partition.

According to /var/log/apt/history.log.9.gz, I've been on initramfs-tools 0.120 since April 2015, so I don't think this is the problem.
I tried sudo apt-get install initramfs-tools=0.116, and got error message "E: Version '0.116' for 'initramfs-tools' was not found."
