pop server ->fetchmail->exim->mail queue->exim->procmail->some mailbox
      ^^^^^^^^^^^^^^^^^^^^^^^^^^^^  ^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^
                    A                             B
'A' happends when you run 'fetchmail'. After 'A', 'B' happends. After
'B' finishes, all your messages are ready to be read in Mutt. If you do
not wait for 'B' to finish, then you will get 'new mail' while in Mutt.

If you 'tail -f /var/log/exim4/mainlog' before 'fetchmail', you will see
this happen.
