I asked the wrong question. The correct question is "How do I teach KDE 
about xv?"

Click on the K button -> Settings -> Menu Updating Tool ->scan. Click on
"xv". Then K -> Settings -> Menu Editor ->Graphics -> xv. Set Command to
"xv -vsm %F /my/top/image/dir".
