    - if the disk is bad ... all /dev/mdxx should look
    the same ...
did you do raidhotremove, raidhotadd of the /dev/hda from each partition ( /dev/md0 ... /dev/md2 )

make raid in degraded mode

transfer your stuffs to the raid array, and then synchronize(rebuild) the array
