In most cases, you just use dist-upgrade; If you do a huge upgrade, like woody>sarge or sarge>sid, then it might be smarter to first do the "safe" part (apt-get upgrade) and then the harder part (apt-get dist-upgrade) I read somewhere apt-get is not the recommended way for this but it worked fine for me last time I tried ;)
