It uses the installed MTA and passes the message on the command line,
from the muttrc man page:

   sendmail
      Type: path
      Default: "/usr/sbin/sendmail -oem -oi"

      Specifies the program and arguments used to deliver mail sent by
      Mutt.   Mutt  expects  that  the  specified  program  interprets
      additional arguments as recipient addresses.

The item you're looking for is:

   mailboxes filename [ filename ... ]
      This  command specifies folders which can receive mail and which
      will be checked for new messages.  When changing folders, press-
      ing space will cycle through folders with new mail.
