First, apologies for responding to my own post.  I don't have this
solved yet, but I did find that I was using the wrong README in
/usr/share/doc/amavis-exim.  I was using the most recent, which is for
exim v4, but my exim is v3.36, so I'm now reading README.exim.old, which
is different.  I've made all the changes that are recommended in
README.exim.old, and now in /var/log/exim/paniclog I get:

2002-11-17 12:53:01 Exim configuration error
  router amavis: cannot find router driver "manualroute" in line 386

Now I'm really confused, because in my exim.conf, line 386 is a comments
line.  Hmmm....

I'll keep trying!
