Who gets to choose what is included?

If you would be so nice to take the time to find out how debian works
you wouldn't make a statement like this.
It's not like everybody that suceeds in making a .deb gets into the
distro.
If you don't appreciate the fact that the developers strive to to give a
lot of choices to dummys like me debian or linux might not be for you.
It's like everything else - you can't make a choice if you don't know
what you want.
