I tried running dpkg-reconfigure xserver-xfree86 because I noticed that I had not supplied the video card ID and monitor ID. But the problem still exists. Has anyone had a similar problem? Any thoughts on how to fix it?

You have done all the right stuff, just didn't get into the "detail" you needed. If you re-run "dpkg-reconfigure xserver-xfree86" and select the "advanced" option about half-way through, you will get some extra screens where you can define your horizontal and vertical scan rates of your monitor rather than accept the defaults given by the other routes. This should fix you up. If you already have a working config and just need to "fine-tune" it, as you appears you do, then your previous config options will appear as the "defaults". You can accept these values until you get to the proper section where you are asked about using the "advanced" config options..

IMHO, going the "dpkg-reconfigure" route is the best way to get X going in the 4.X versions. Beware of the "use framebuffer" question... I have had a LOT of problems when answering "yes".
