I had the same problem after upgrading exim to the last version and fixed it just like you did. After that I noticed that there is already a bug filed, so didn't bother to dig in it any deeper... But now i did, just looked in the postinst script and noticed that it 'upgrades' the databases to version Berkley DB 3 with db3_upgrade. Maybe that's got something to do with it?
