this would be SecurID or similar.

S/Key is a list of passwords generated based upon a seed password and
an initial value

  otp-md5 128 fl6641 ext

tells you that it wants password #128 off the list that was
initialized with fl6641, in an md5 encrypted fashion.

compare s/key to the TAN numbers used for electronic banking in parts
of europe. it's *not* SecurID.
