What I have done is put 

audio/x-pn-realaudio; /usr/bin/X11/rvplayer %s

in ~/.mailcap and 

type=audio/x-pn-realaudio  \
desc="RealAudio"  \
exts="ra,ram,rm"

in ~/.mime.types and then restart Netscape.
