xmlto, when used to build FO files created from Docbook-XML by xmlto,
fails on any file containing <itemizedlist> or <orderedlist> tags.
This makes it apparently impossible to build Docbook using this
toolchain.

I'm not sure that this is an xmltex bug rather than a docbook-xsl or
xsltproc; if it is please excuse me.

This simple file demonstrates the problem:

On my machine, this fails with:
