When $HOME is empty. For example, as root:
# /usr/bin/env - /usr/bin/texhash
texhash: Skipping /etc/texmf
texhash: Skipping /usr/local/share/texmf
texhash: Skipping /usr/share/texmf
texhash: Skipping /var/lib/texmf
texhash: Done.
