If I understand your message correctly, the error unwind procedure that
is being applied here is the one after:

  <old-postrm> upgrade <new-version>

failed, in which case the cleanup action performed is:

  <new-postrm> failed-upgrade <old-version>

That might also be:

  <new-postrm> abort-upgrade <old-version>

in case of several errors.
