The current version of examdesign, available from
http://ctan.tug.org/tex-archive/macros/latex/contrib/examdesign/
requires you to say \usepackage{examdesign} in your LaTeX document.
It appears from what you have shown that you have \usepackage{exam} in
your document instead.  Might that be the cause of the problem?

Alternatively, if you have an older version of examdesign installed on
your system, where is this installed?
