Plug in an Ethernet card, add a section in /etc/network/interfaces for eth1. If it sets up the cards in the wrong order then remove the entries for eth0, eth1, eth1_rename etc. from /etc/udev/rules.d/70-persistent-net.rules.

I think that you get eth1_rename when the OS changes its mind about the order of the card allocation.
