I suggest using jade. I find that wrapper scripts tend to hide or
obscure problems.

Hum! I guess not. Research for this problem on the Internet shows me
the xml.dcl we shipped in sgml-data was wrong and thus caused a
problem with this. Update your sgml-data package from unstable (1.8
or better, will be there shortly) and try again.
Are doing this, I was able to process the file, like so:
SP_ENCODING=xml openjade-1.4devel -E1 -t sgml -d
../../dsssl/html/docbook.dsl /usr/share/sgml/declaration/xml.dcl
mathml.001.xml
