Please include the following example of ~/Mail/scan.form
in /usr/doc/mh/examples.

This setting is needed for Japanese, because
headers written in Japanese are encoded by MIME.
I propose the file name to be '/usr/doc/mh/examples/scan.form.MIME'
and a /usr/doc/mh/examples/README file should be prepared that writes:
