I will close this bug.
To be able to use the field file name, you need to insert the picture as
a link. This is an option from the dialog Insert Picture. The default is
not to insert the image as a link.

In v 2.4 of OOo writer, the field "file name" is clearly in a section of
the dialog marked "link".

If you use default options when inserting an image, then it is not a
link, and the "file name" field  is correctly disabled.
