In debian/control, you should start the short description with a lower case (bandwitdth limiting module for apache2). You compare your package to libapache2-mod-cband. However, this package has been removed from Debian. You should elaborate more on the features of mod-bw without making comparison with mod-cband. debian/copyright contains the full Apache 2.0 license. This license is contained in common-licenses package. You should point the user to /usr/share/common-licenses/Apache-2.0 instead. You packaged generated file debian/substvars. You should remove it. You can use dpkg-buildpackage -S to cleanly build the source package.
