I personally prefer to follow the GNU coding standards here [1] for
patch descriptions.

diff can also print the C function as extra information in the diff. But
I'm not sure, if this is enabled by default in quilt and if it works for
more languages than just C. So manual descriptions are IMHO a good
thing. However, they increase the workload, so decide yourself.
