[PATCH] use new switch to apply live appends to LH_BOOTAPPEND_INSTALL as default

I think this is a cleaner way of setting the default for
LH_BOOTAPPEND_INSTALL, I think it is problematic when you
place variables inside of variables as you are not ensured
that the values will get substituted appropriately.
