In the case of quilt I see 2 possibilities for the future:

1) 3.0 (quilt) format

I think if that is used then no readme is needed.

2) manual patching with quilt in debian/rules

I think that will become uncommon use of quilt once 3.0 (quilt) format
is accepted by the DAK. And then it should have a readme again.
