Each of the two suggested notices will be fine with the DFSG, except..

This one is probably fine, except that there is precedence (pine) that
some copyright holders expect "copy, modify, and distribute" to mean
"you may modify AND you may distribute, but you must not do both at
the same time".

To steer clear of that risk, just replace "and" by "and/or".
