I have the same behavior, since plasma 5 is using pulseaudio, the management of the volume became crazy...

Sometime without any reasons, volume is going up to 100%... but most of the time is it :
- When system sound is played (regression of that bug : https://bugs.kde.org/show_bug.cgi?id=324975)
- When session start,
- When restoring session from standby mode...

I think plasma 5 is insane to move the volume to 100% !

This is dangerous if you are using earphone and socially difficult to accept when you work in an open space...

I can't find any reason to legitimate the creation of this kind of behavior so, I suspect some side effects...
