As for your first problem... no clue. The second one I ran into today. I did that by enabling "show crypto icons" in Configure KMail:Appearance:Headers. All the messages I hadn't looked at recently (whether flagged as Read or Unread) got two little question marks where the crypto icons were on other messages. Maybe you enabled that setting?
