The problem you pose is to pxying h323
For doing that you need a special proxy cause there a dynamic allocation of
some ports in this protocol.
I have done that before : Use phonepatch (but you need money) or h323_ipmasc
module
you can also try the proxy provide by people wicha are making openh323
project

I have use the module ipmasc and phoneatch. the first one is not very stable
phonepatvh is a good product
