I believe this is fixed in the latest release of mkisofs (and mkhybrid,
as it's based on mkisofs) - I haven't tried it, but comments in the
mkisofs code say:
SVD for El Torito. MUST be immediately after the PVD!
and the El Torito SVD is now written out immediately after the PVD and
before any Joliet SVD.
