Debian Developer Survey (short, anonymous)

As part of my Stat II project at PCC (http://www.pcc.edu), I'd like to
ask for a minute of time from any Debian package maintainers.  The
survey is anonymous, basically asking demographic information that will
be used in class to look for correlations.

The questions:
 * Age
 * Education level
 * Number of source packages maintained
 * Number of binary packages maintained
 * Debian user since <date>
 * Debian developer since <date>

I've put up an html form for responses, or an e-mail would certainly
work.

At some point next week, I'll post the data (CSV, probably) and some
summary results.
