Boot method: I copied the contents of the network install iso file into a
directory (another partition), and then mounted it.

I get: Failed to determine the codename for the release

Then, I followed the instruction of http://www.debianhelp.org/node/8799
replacing etch by lenny. I got a message of success (but there seems to
be no second step).

I am stuck because the installer refuses to configure the package manager.

I don't call this a failed installation to be honest. I call this failed 
abuse of the netinst CD...

I suspect the only actual problem may be a missing hidden directory 
(.disk), but please don't do something that was clearly never really 
intended and call it "failed" if it does not work.
Even if you add the missing directory, things may still fail because the 
code expects that it can mount and unmount the CD at certain points.

If you can show failure when you use the CD as CD, that's fine.
What you should be using for your case is a hd-media image in combination 
with the CD. Please consult the installation guide.
