I think it's because old gtk+ libs did not need cairo which was introduced later on;
Can't remember the details but IIRC the directfb backend was introduced sometimes around
Jan 2006.

I'm afraid it's the second option :)
Shouldn't be too hard though: you can take a look at [1]. I guess that the only important
bit is the "--enable-directfb --disable-xlib" options.
This -dev package, once installed, will be used for compiling gtk+-directfb (u)debs.
