I can read /etc/init.d/apache2 to figure out I need 
/etc/default/apache2.

But if you were going to offer START switch through
/etc/default/apache2, why did not you offer template file with comment
there.  I do not see any file installed there.  

Please add /etc/default/apache2:

These bugs were fixed in NMUs by me which are, apparently, no longer
considered NMUs because I'm now part of the apache2 maintenance team.
