I have just added the information on
http://wiki.debian.org/accessibility :
“For KDE accessibility, the qt-at-spi package needs to be installed,
and the QT_ACCESSIBILITY environment variable has to be set to 1.”
About Skype, note that IIRC it renders its own widgets by itself, so
that it will be completely inaccessible, even if it uses qt for some
other things.
