Probably not without modifying the Orca scripts.

You could try mutt with the braille_friendly=yes option in your ~/.muttrc
configuration. I suppose Alpine would be another, fairly straightforward,
possibility that would work well with BRLTTY if you select the "show cursor"
option in its configuration.
