I had my points increased temporarily to be able to give 150 points for
people at this year's FISL but the points were decreased again like a
week ago. I've mailed them requesting extension for that so if they do
accept my request I'll be able to give 150 points for people who can
identify themselves with 2 official documents.

In which case only one form is needed.
