With official use cases, this kind of media will not be usable.

The only exception doe a CD with a new session not starting at 0 is
a CD+ (audio at session 0, data at session 1+).

Noted. But not all useful things are "official," and vice versa. One of the most useful features of mkisofs is -U, as an example, to save original data without having to create an ISO with a tar file or ext3 image.
