Start with a clean setup:

create the image with  a _recent_ mkisofs

ftp://ftp.berlios.de/pub/cdrecord/alpha/

and use mkisofs -dvd-video to create the image

use cdrecord to write to the medium

If the DVD-RW medium has been written by growisofs before
you need to cleanup the medium with cdrecord -v blank=all before.


Make sure you use the official and unmodified cdrecord  and not 
a bastardized version from one of the Linux distributors.
