The PDF form was created (Creator) by `Adobe PageMaker 7.0' and produced 
 (Producer) by `Acrobat Distiller 5.0.2 for Macintosh',
and I used `acroread'.

This is the case here:
note that the fields to be completed were fields meant to be filled
(I do not know if one can do such field with LaTeX :-()

 but we don't know what
