That sounds like you had a form in another format and were able to edit
the form with the native application.

The PDF form was created (Creator) by `Adobe PageMaker 7.0' and produced (Producer) by `Acrobat Distiller 5.0.2 for Macintosh',
and I used `acroread'.

organization provides a form in a PDF,


This is the case here:
note that the fields to be completed were fields meant to be filled
(I do not know if one can do such field with LaTeX :-()

 but we don't know what
application made the form nor do we have the source document?  It is in
this case where I would like to be able to edit the PDF form directly.
