I'm using the following rule to incorporate clamscan directly into
my .procmailrc:

:0HB
* ? clamscan --mbox --quiet -; if [ $? == 0 ]; \
then /bin/false; else /bin/true; fi
spambox

(note that the second and third line should be one)
