What does "ls -l /dev/dsp" say?
Did you log out and in again?
What does groups say?

Btw, adding a user to all groups is a bad idea. Being in the disk group
for example allows you to overwrite your hard disk partitions.

I have no idea. Do you get any output when you run konqueror from the
command line? Did you check if the process is maybe still running and
somehow preventing new instances of konqueror to load? Check if some
processes are still running with "ps". Look for konqueror and kio.
