There was a small discussion in #215925. The author of index.sty has
uploaded a new version to CTAN:macros/latex/contrib/index . The above
described behaviour is not visible any more. Thomas has confirmed,
that he has included the new version into teTeX. Hence I tag your bug
fixed-upstream.
