On my Macbook Pro, kmix's icon in the taskbar, when clicked, always 
wants to control the headphone volume only.  I'd like it to instead 
control the PCM volume, but there is no way to tell it to do that.  
Moreover, when I try to tell it to map certain hotkeys to PCM, it is 
ignored; my volume up/down buttons stay mapped to the headphone volume.

I suppose you're clicking over the kmix icon always with the left button
of your mouse. If you click with the right button instead, you'll have a
different menu where you do exactly what you are asking for.
