On a fresh installation, we logged in as a normal user to find that
/tmp/.ICE-unix was owned by the user logged in. Subsequently the
ksmserver failed to start until I chown'd the directory back to 0:0.

/tmp/.ICE-unix is created in sarge with xfree86-common package,
inittime. If this isn't set properly (which I doubt is the case 
anyomre), it would be a bug there.
