I've tried adding that line as well. If I set it to 16, the icon
appears smaller, but the offline icon still isn't rendered. If I set
it to 22 then it still displays a blank page for the offline icon.

I had some not upgraded. I've upgraded them now and restarted KDE,
and the icon still don't work.
Now left are (not libmimelib1 is removed):
