I did some googling.

/etc/kde3/ui/ui_standards.rc was missing (indeed the directory was
completely empty).

I reinstalled kdelibs-data and then:
  * that file was now present
  * I got a full set of menus

I notice that /etc/kde3/ui/ui_standards.rc is listed as a conffile.
Perhaps this has something to do with why it was not installed
properly?  Could it have been a conffile of some other package since
removed?
