Bug#257101: reply-to is ignored if mail-followup-to is set

Plain reply uses M-F-T, Reply to author uses R-T, Reply to all uses
both, and Reply to Mailing List uses m-f-t as well. I find this mildly
confusing, but however other people might and will think different.
Please have the behavior configurable.
