OpenOffice.org Base crashes in a database form (NewItemForm in database to be attached) on trying to save a record.  It 
looks like there is a dialogue box trying to come up, but it never appears, and OOo is complete frozen and all soffice stuff 
must be killed to relaunch OOo.  Cancelling recovery gives back the database less the last time data was saved while trying 
to recover the database results in missing forms.

This may be related to the odd way the form windows display (I'll have to do screen shots in another bug report), 
specifically that there are dual title bars, offset windows inside the Writer window (for forms), and other strangeness.

I opened the database document attached to the bug report, and enabled macros.
I violated index conditions on the subform in the form "Category Maintenance" by attempting to enter a record with a index value already existing
OOo-base behaved as expected: a dialog box informed me of the key-violation.
