Basically, no diff is taken from between when there is no translation and the first translation. I thought showing diffs from a field full of <trans> to actual text looked a bit silly. If you press "accept with changes" but don't actually change anything it preserves the old diff. So no, the diff doesn't get lost just because someone pressed the wrong button.
